Seven Hills Foundation Supporting Opportunities for All Abilities
Seven Hills Rhode Island (SHRI) offers a continuum of services for children and adults with disabilities and life challenges. SHRI promotes health, well-being, and active lifestyles for all. Our person-centered programs foster individual potential and include:
n Early Intervention for children, birth to age 3, who are experiencing, or at risk for, developmental delays.
n Home-Based Therapeutic Services (HBTS) intensive supports and treatment for children with disabilities.
n Personal Assistance Services and Supports (PASS) assistance with daily life skills for families of children with special health care needs.
n Day Habilitation integrated occupational, physical, and speech therapy along with nursing and behavioral supports on-site or in the community. Participants receive training in functional life skills, health maintenance, fine- and gross-motor skills, self-direction, socialization, and communication.
n Residential Programs for adults with developmental disabilities, autism spectrum disorder, brain injury, and complex medical and/or behavioral challenges.
n My-Choice authorized fiscal intermediary program to promote independence by empowering people with disabilities and their families to self-direct and control their supports, services, and lives.
n Community Supports day programs and sports, recreation, & leisure activities for adults with disabilities.
n Conflict-Free Case Management for participants of Medicaid Home and Community-Based Services (HCBS)
to express preferences and make choices about living options, community activity, socialization, and more.
n Employment Matters and Workforce Readiness & Employment pathways to competitive employment that include vocational assessment, training, and job placement for adults with disabilities.
